If you need a vital record, please mail a request with the appropriate fee or visit our office in-person. For additional information see the information for births and deaths below. Marriages are by appointment only, please call us at 860-584-6200 ext. 0.

Birth Certificates

A copy of your birth certificate can be obtained from the Town Clerk’s Office in the town where the birth occurred or in the town in which the mother resided at the time of birth. The office can also issue certified copies of birth certificates for residents born in any Connecticut town after January, 2002.

A certified copy of a wallet-size (short form) birth certificate is $15. A certified copy of a long form birth certificate is $20. Out-of-state checks and debit or credit cards are not accepted.

You may request a birth certificate by mail by sending a request form, the fee and a copy of a current government issued photo identification. For additional information please review the statutory guidelines. Birth records less than 100 years old may be accessed by the following:

  • The person whose birth is recorded (if person is 18 or older)
  • Spouse
  • Parents
  • Children (if child is 18 or older)
  • Grandchildren (if grandchild is 18 or older)
  • Grandparent
  • Legal guardian
  • Persons authorized by court order
  • Attorneys and title examiners representing the person or such person’s parent, guardian, child or surviving spouse
  • Agents of a state or federal agency as approved by the Department of Public Health

Beginning July 1, 2021, a person can request an uncertified copy of the original birth certificate of an adopted person. The form would need to be completed along with the appropriate documentation and the fee of $65. The instructions specify the requirements. 

Death Certificates

A death certificate can be obtained from the Town Clerk’s Office in the town where the death occurred or the town where the deceased resided at the time of death. A certified copy of a death certificate is $20. Out-of-state checks and debit or credit cards are not accepted.

Marriage Certificates

A copy of your marriage license can be obtained from the Town Clerk’s Office where the marriage occurred or your town of residence at the time of marriage. A certified copy of a marriage license is $20. Out-of-state checks and debit or credit cards are not accepted.
